Have your say and join our Patient Participation Group - for more details contact Reception!

We have a number of different clinicians working with us, GPs, Practice Nurse, Health Care Assistant, Health Improvement Practitioner, Pharmacist, a Mental Health Practitioner & coming soon a MSK Practitioner - to help us book you in to see the correct person - Reception will be asking you what your appointment is regarding, this is to ensure you are booked with the correct person and gives them time to prepare for your appointment.

Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete a travel vaccination form (1 per patient) and bring this to the surgery.  The Practice Nurse will review the form to understand what vaccines are required for that country and will check your vaccination record to determine if vaccinations are required.  Please call into the surgery for a form or you can print the form below, complete the information and hand to one of our receptionists.

It is important to make this initial enquiry as early as possible - at least 6 weeks before you travel - as an appointment may be required with the Practice Nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

There is further information about countries and vaccinations required on the links below:
